He was predeceased by : his father Bobby Ray Maddox; and his grandparents, Robert, Lucile Phillips, Lathan and Frances Maddox.

He is survived by : his wife Dianna Maddox; his mother Connie Maddox (Ricky); his daughter Heather Smith (Josey); his grandchildren, Waylon and Whitley Smith; his nephews, Gavin Lawler and Carter Ledbetter; and his niece Jaylynn Fortenberry.
