She was predeceased by : her husband James McCoy Formby; her son Gerald Wayne Formby; her parents, Norris Davis and Redith Davis; and her sister Dorothy Vanderford.
She is survived by : her daughter Diane LeCroy (Phil) of Piedmont; her grandchildren, Jimmy LeCroy of Piedmont, Cassie Holbrooks (Wesley) of Piedmont, Carisa Sparks (Clay) of Huntsville and Mark Formby (Brittany) of Southside; her great grandchildren, Rylie Anne Holbrooks, Owen Holbrooks, Deacon Sparks, Dawson Sparks, Davis Formby and Elsie Formby; and her daughter-in-law Brenda Formby of Huntsville.
In Lieu of flowers the family suggest memorial contributions to Goshen United Methodist Church.
