He was predeceased by: his parents, Walter Samuel Whitehead and Elizabeth Ann McCoy Whitehead; the mother of his children Phyllis Ann Brown; his wife Patricia Gene Edwards; his daughter Beth Ann Bozarth; his stepchildren, Linda Jennings (Bruce), Michael Robertson and Garril Mixon (Donald); his stepfather Ted Masters; and his siblings, Marie Masters Peiffley (William), John Whitehead (Karen) and Louise Masters Arentz (Chuck).
He is survived by: his children, Steve Whitehead (Terri) and Connie Hurston (Len); and his grandchildren, Gage Bozarth, Alex Whitehead, Nicholas Whitehead (Natalie), Benjamin Whitehead (Kaeleigh), Jackson Hurston (Katherine), Logan Hurston, Kevin Vaughn, Samantha Cook (Jay), William Rouse (Mary Beth), Callie Robertson and Jacob Mixon. He is also survived by great-grandchildren, nieces and nephews.
