He is survived by: his wife Tammy Blake; his children, Phillip Blake (Janice), Anthony Blake (Kristian), Joshua Blake (Tim) and Mary Colburn (Tracy); his grandchildren, Jaxson Blake and Annabel Colburn; and his brother Christopher Blake (Laura). He is also survived by several nieces, nephews and extended loving family and friends.
