He was predeceased by : his parents, John "Dub" William Mayfield and Mildred Pauline Gray Mayfield; and his brothers, Donny, Randy and Billy Joe Mayfield.

He is survived by : his nieces and nephews, Amber Mabe of Altus, Autumn Hunt of Ozark, Arkansas, Nate McMillian of Denning, Scotty Mayfield of Ozark and Justin Mayfield of Altus; and his great-nephew Ethan Mabe.
