He is survived by: his wife Cynthia; his parents, Ann Cook and Mike Cook; his siblings, Carrie Guice (Scotty), Rebecca Fowlkes (Stephen II) and Clint Cook; and his close friends, Dickie, T. J. and Keith. He is also survived by eight children, fifteen grandchildren.
