He is survived by: his sons, Jeremy Middleton and Justin Ertel; his stepdaughters, Mindy Dawn Smith and Ashley Nicole Smith; and his siblings, Shirley LaCore, Jane Story and William Gene Ertel. He is also survived by 3 grandchildren.
