He was predeceased by: his nephew Ian Girouard. He is survived by: his mother Joyce Fultz (Sparks) of Berryville; his wife LeeAnn Sisco Fultz; his daughters, Kami Fultz, Addie and Allie Ashford of Berryville; his father Gerald Fultz; and his sisters, Teri Girouard of Pea Ridge, Sheri Bostwick (Donnie) of Oklahoma, Leonda Davis (Jeff) of Berryville and Leanna Britt (Todd) of Berryville.
