He was predeceased by: his parents, Dennis Cook and Judy Cook; and his brother Brian Cook. He is survived by: his wife Toby Cook; his children, Matthew Cook, Kaylee Smith (JD) and Lindy Cook; his siblings, Shaun Cook, Todd Cook (Roberta), Dan Cook (Tana) and Bonnie Covington (Ira); and his grandchildren, Jaxon Smith, Joanna Smith and Jase Smith.
