He was predeceased by: his parents, Nile Smith and Oleta Hooks Smith; and his son Phillip Wade Smith Jr.. He is survived by: his wife Brenda Barton; his brother Eddie Smith (Rhonda); his children, Zach Smith (Lindsey), Jason Miller (Heather), Nalley Smith and Jennifer Sexton (Kelly); his grandchildren, Eli Smith, Mary Kate Smith, Olivia Smith, Abigail Smith of Dylan Sexton, J. P, Miller Jr., Zachary Miller and Whatley Miller; and his friend Eddie Webb.

In lieu of flowers memorial donations may be made to Team Korie Foundation, St. Jude Children's Hospital, or UCAPS.
