He was predeceased by: his parents, A. J. Crumley and Joyce Seale Crumley; his granddaughters, Elizabeth and Victoria Crumley; and his siblings, Joyce Marie Kattner (E. C.) and Grafton Seale.

He is survived by: his stepmother Elna Crumley; his wife Mary Kay Allen; his family Boy Scouts; his children, Allen Crumley (Rebecca), Casandra Cox (Jody) and Brett Crumley (Eliese); his grandchildren, Cade Cox of Northwest Arkansas, Catherine Crumley of Northwest Arkansas and Makayla Cox of Northwest Arkansas; his brother Dub Crumley (Bonnie); his brothers-in-law, John Doyle Allen and Jerry Allen (Esther); Charolette Seale of Texas.

In lieu of flowers, the family kindly requests donations be made to Post-Polio Health International in memory of Lewis Crumley.
