He is survived by: his wife Kimberly Velez; his children, Michael Velez (Kathryn) of Fort Smith and Rebecca Smith of Fort Smith; his sisters, Haydee of Puerta Rico, Angela of Puerta Rico, Aida of Puerta Rico and Ester Velez of Puerta Rico; and his grandchildren, Kylie, Lillian Velez, Jack, Emilee and Claire Smith.
