He was predeceased by: his parents, Reece Shook and Dot Shook; his siblings, Edna Raye Moore and Reece Shook Jr. (Butch); and his son-in-law Gary Collison.
He is survived by: his wife Janie; his children, Paul Shook (Alina), Sue Leavell (Todd), Teresa Collison, Randy Johnson (Jackie) and Jeff Johnson (Lori); his grandchildren, Abby (Andrew), Garrett, Leslie, Jeff, Josh (Bailee), Jordan, Kace, Lance, Dalen and Hunter; his great grandchildren, Ella, Brooks, Kreed and Layklee; and his sisters, Debbie Marsh (Steve) and Becky Shook.
In lieu of flowers the family requested donations to be made to the American Cancer Society.
