He is survived by: his wife LaVita (Burke); his children, LaResa Polk of Hickory Plains, Bryan Polk of Hickory Plains and Sheryl Brannon (Gary) of Hickory Plains; his grandchildren, Jarret Polk, Nicholas Polk (Maggie), LaBeth Polk (Brandon Garringer), Darius Milam (Chrissy), Keith Polk and Jordan Morris; and his great grandchildren, Kaleb Pierce, Addison Ritchey Milam, Liam, Charlotte McKendree and Kaidyn Brannon. He is also survived by many nieces and nephews.
