He was predeceased by: his parents, Claude Lewis and Madge Lewis; and his son Jack. He is survived by: his siblings, Tim (Ann), Maurine (Buddy), Joel (Emma), Laura and Cheri (Scott); his sons, Thomas and Robert; and the mother of his children Mary. He is also survived by his brothers and sisters and many nieces, nephews and cousins.

In lieu of flowers, the family requests memorial gifts be made to the Jack Lewis Memorial Fund at the Arkansas Community Foundation.
