He is survived by: his mother Barbara Adams Kozak; his wife Joni Price Kozak of London, AR.; his siblings, Patrick (Kaye), Timothy (Terry), Kevin Kozak and Ileen Haight (Sean); his aunts, Lucille Vogel of Pennsylvania and Pauline Adams of Pennsylvania; and his children, Michael Kozak Jr. (Shay), Brandon Kozak of Texas, Kaylan Draper (Troy), Connor Whitted, Bailey Hughes (Dylan) and Wyatt Jackson. He is also survived by Numerous grand-children and great-grand children, nieces and nephews.
