He is survived by: his parents, Kelley Grier of Florida and Steven Samples of Lowell, Arkansas; his daughters, Kylee Samples of Springdale and Hailey Guy of Springdale, Arkansas; the mother of his children Kerry Guy of Springdale, Arkansas; his brothers, Tyler Samples of Springdale, Steven "Cody" Samples of Polk County, Scott Samples of Oklahoma and James Samples of Colorado; his grandmother Mary Turner Lackey; his aunts and uncles, Pam Soto of Orlando, Roy (Pam Samples), Tim (Kelley Samples), Kathy (Jack Russell), Cheryl (Gary), Johnnie, Ivan and Patricia Crawford of Springdale, Arkansas; and his cousins, Bill and Corena Fields of Springdale, Arkansas.
