He was predeceased by: his parents, John Perry and Dorothy Jack (Aldridge); his grandparents, Douglas Houston, Tommie Jack (Keith), Herbert and Wilma Aldridge (Bridges); and his aunt Eloise Killingsworth.

He is survived by: his brothers, Scotty Jack (Susan) and Michael Jack (Angie) of Magnolia; his fiancée Jeanne Haltom of Magnolia; his nieces and nephews, Jasper Jack (Sherry), Morgan Bailey, Brandon Jack of Magnolia and Cory Jack of El Dorado; his friends, Sydney and Hunter Grubbs of Hot Springs; his great-nieces and great-nephews, Landon Smith, Abigail Jack, Adalynn Grubbs, Dawson Jack and Deacon Jack; and his aunt Katie Long of Magnolia.
