He is survived by: his mother Eva Jean Maddox Phillips of Marked Tree; his wife Jackie Phillps; his children, Jerry "Bubba" Phillips (Shelbie) of Marked Tree, Ashley Ann Phillips of Covington and Cortney Jean Welander of Cincinnati; his brother Scott Edward Phillips of Marked Tree; and his grandchildren, Nikki, Elena, Grant, Laikyn and Weston.
