He was predeceased by : his parents, Glyn Polk and Montine Sledge Polk; and his brother Glyn Allen "Bud" Polk. He is survived by : his sons, Nathan Polk of Morrilton and Aaron Polk (Cornelia) of Guy; his stepsons, Jeffery Leon (Katia) of Morrilton and Lassiter Leon (Kelsey) of Morrilton; his grandchildren, Braxton and Milayah; his sister Linda Roberts (Pat); and his sister-in-law Marilyn Polk.
