He was predeceased by: his parents, Charles Dolezal and Joan Dolezal (Berish). He is survived by: his former spouse Miss Melinda Smith; his wife Melinda Dolezal (Smith); his daughters, Lauren Harbourt (Tyler) of Conway and Katie Daigle (Caleb) of Knob Koster, Missouri; his grandsons, Maddox Harbourt, Camden Harbourt and Mason Daigle; and his siblings, Dr. Edward Dolezal (Carolyn), Laurie Bolanowski (Ronald) and Karen Dombrowski (Don). He is also survived by nieces and nephews; and a lifetime of friends.
