She was predeceased by: her husband Bobby Forkum; her parents, Jack Deen and Lillie Deen; her granddaughters, Reianna Forkum and Destiney Forkum; and her brothers, Carl Head, Homer Head and Alfred Deen.
She is survived by: her sons, Ricky (Karen) and Terry Forkum (Tammy).
Visitation will be held on Thursday, June 5th 2025 from 11:00 AM to 1:00 PM at the Heath Funeral Home (321 W Garland St, Paragould, AR 72450). A graveside service will be held on Thursday, June 5th 2025 from 1:00 PM to 1:30 PM at the Greene County Memorial Gardens (2901 AR-135, Paragould, AR 72450).
