He was predeceased by : his parents, Leeman Formon and Rachel Formon; and his brother Warren Formon. He is survived by : his wife Rebecca Watkins Formon; his children, Amee Tribble (Shannon) of Trumann, AR, Derek Formon (Stacy) of Paragould, AR and Andy Formon (Misty) of Paragould; his grandchildren, Kellie, Allyson, Addison, Kadence, Landon and Riley; and his sister Julie Formon of Leachville, AR..
