He was predeceased by: his parents, William Henry and Lucy Sims Cook; his siblings, Bill Cook and Barbara Starks; and his stepdaughter Alicia Taylor.

He is survived by: his wife Patricia Cook; his children, Shelly Cook and Charles "Tony" Cook; his granddaughters, Eden Cook and Hillary Bogus; his stepsons, Jeff Loveless and Chris Wilson (Cassey); and his nieces, Melanie Turpin (Joe), Patty Behnke (Jay), Robin Pfeiffer (Dwayne) and Barbara Arbutina (Lucas). He is also survived by 3 great-grandchildren, eleven step-grandchildren; four step-great-grandchildren.

Memorials can be made to the Fairview Event Center (Old Fairview Gym), in honor of Charles Cook, or an organization of your choice.
