He was predeceased by: his parents, Tommy and Isabella. He is survived by: his children, Joseph Forrest, Calvin Forrest and Teresa Forrest of Pine Bluff; and his siblings, Tommy Forrest, Jr., Jonathan Forrest (Cassandra) of Pine Bluff, Ruthie Madden (Jimmy) of Pine Bluff, Della Ferguson of Pine Bluff and Jamie Rouse (Wright) of Pine Bluff. He is also survived by nieces nephews and other relatives and friends.
