He is survived by: his wife Linda Cathey Formby; his son Phillip Wayne Formby (Glenda); his son-in-law Dow G Wright; his grandchildren, Jonathan Trosie Formby (Heather), Brady Gillam Wright (Allison), Mark William Wright and Lily Elizabeth Formby Bruce (Taylor); his great grandchildren, Emerie Grace Formby, Mila Hope Formby and Noah Jax Formby; his brothers, Carlton E Formby (Betty), Max Harold Formby (Karen Ann), Mike Puckett and Neal Ridling; and his caregivers, Pam Morrow, Steve Blevins and William. He is also survived by numerous nieces, nephews, a host of extended family, and friends.
