He was predeceased by: his parents, Paul Celton Polk and Hazel Olive Polk. He is survived by: his sisters, Paula Osteen and Debbie Gross; his brother-in-law Terry Gross (Sharon Gerg); and his nieces and nephews, Kerri, Christy, Kim, John Paul, Samanthajoe, Stephen, Cassie and Mandy. He is also survived by many great-nieces, great-nephews, extended family members, and dear friends.
