He was predeceased by: his parents, Warren Shade and Jaunita Shade (Brown). He is survived by: his wife Rhonda Shade; his children, Savannah Evans (Ben) of Greenland, AR and Sam Shade (Shelby) of Fayetteville, AR; his siblings, David Shade (DeeDee) of Bentonville, AR, Denise Eoff of Farmington, NM, Vicki Zook and Molly Sanders (Jimmy); his family, Aaron, Stephen, Amy, Tiffany and Chad; and his father-in-law Ronnie Apple. He is also survived by many nieces, nephews and cousins.
In lieu of flowers the family ask that donations be made to Monarch 61.
