He was predeceased by: his father Butch Diaz. He is survived by: his daughters, Jealousy (Alonzo) and Gabby; his grandson Alonzo; his mother Joann Lopez Diaz; his siblings, Jessica (Marcus), Joshua (Dani) and Jaime; and his son Jacob Jr..
