He is survived by: his parents, Kevin Moline and Cindy Moline; his fiancée Hope Burbes; his aunts and uncles, Jeff Fox (Paula), Debbie Seipp (Reid), Mary Schaefer (Michael), Jimmy Fox, Tammy Bacon (John) and Rich Hughes (Toni); his cousins, Ryan, Ashley, Alex, Max, Josh, Dominick, Madison, Samantha, Megan, Alison, Daniel, Tracey and Becky; and his pet Ace. He is also survived by his parents, sister.
In Lieu of flowers, family and friends are invited to make a memorial donation in Jay's name to the following organizations close to Jay, Hope and their families hearts. Camp Rhythm, St Louis Children's Hospital Foundation.
