He was predeceased by: his parents, Archie Chouinard and Marie Chouinard (Schlumpberger); his former wife Marlene Ann Chouinard (Buhmann); his wife Patricia Chouinard (Tishman); his siblings, Donald Chouinard and June Lodge; his son Steven Chouinard; and his stepchildren, Robert Callahan and Donna Chrysokas.
He is survived by: his children, Nannette Chouinard of Hereford Az, Suzanne Scardemaglia of St Charles, Il and Gregory Chouinard (Cathy) of Hereford, Az; and his stepchildren, Cheryl Callahan of Hereford, Az and Steven Callahan (Anne) of Queen Creek, Az.
The Foundation has now expanded to included a prayer house and La Purisima Retreat Center.
