He was predeceased by: his mother Barbara Smith; and his grandmothers, Eldina Black and Faye Frank. He is survived by: his father Allen Collins; his children, Maylee and Jace; and his siblings, Jason Forque, Toni Martinez and Lindsey Davis. He is also survived by his many nieces and nephews.
A funeral service will be held on Monday, November 2nd 2026 at 10:00 AM at the Nw Valley Baptist Church (4030 W Yorkshire Dr, Glendale, AZ 85308).
