He is survived by: his wife Carol Ann Folker; his children, Dennis (Karen), Carrie, Denise (Kevin), Lisa (Todd) and JoAnn (Kris); his mother Geraldine; his sisters, Susan and Linda (Mike); and his grandchildren, Jared, Chase, Ashley, Derek, Damin, Eli, Kloe, Sofie, Katie, Krissy, Michael, Nicki, Brian, Collin, Alyssa and Daniel. He is also survived by many nieces, nephews and friends.
