He was predeceased by: his sister-in-law Whitney. He is survived by: his parents, Janis J. Smith and Michael S. Smith; his wife Amy; his children, Skye Michelle Smith, Sherrod Travis Smith Jr. (Brooklyn), Shea Joseph Smith (Sahya) and Sage Leanne Smith; and his siblings, Janette Booth (Ken), Todd Smith (Jennifer), Trevor Smith (Natalie) and Tyson Smith (Brianna). He is also survived by many extended family members and dear friends.
