He was predeceased by: his parents, Joseph D'Amato and Anna D'Amato; and his siblings, Nicholas, Joseph, Nancy and Rosemary.

He is survived by: his wife Patricia D'Amato (Melton); his children, Veronica Torgerson and Thomas D'Amato Jr; his stepchildren, Kimberly Cochrane (William) and Michael A. Piche III (Jessica); and his grandchildren, Mia Cochrane, Michael P. Piche and Matthew Piche.

In lieu of flowers the family ask donations may be made to the East Valley Hospice1311 W Chandler Boulevard, Chandler, Arizona 85224.
