With solemn hearts, we announce the passing of Jade Antonia Self of Phoenix, Arizona, whose presence will be deeply missed, having departed on June 9, 2025 at the age of 31. Family and friends are welcome to send flowers or leave their condolences on this memorial page and share them with the family.
As we bid adieu to Jade Antonia's earthly form, let us not dwell on the void left in her wake, but rather celebrate the legacy of love and inspiration Jade Antonia has bequeathed us. For though she may have departed from our sight, her spirit remains a timeless beacon, guiding loved ones through the labyrinth of life's journey.
She was predeceased by: her father Theodis K. Self; her daughter Ariana; and her brother Jeremy. She is survived by: her mother Carla Marie Vasquez; her children, Jayden and Aaliyah; and her siblings, Jasmine and Jared. She is also survived by nieces, nephews, aunts, uncles, cousins and friends.
Viewing will be held on Wednesday, June 25th 2025 from 9:30 AM to 10:30 AM at the Blessed Sacrament Church (512 N 93rd Ave, Tolleson, AZ 85353). A Mass will be held on Wednesday, June 25th 2025 from 10:30 AM to 11:30 AM at the same location.
