He was predeceased by: his wife Chris Kuhl Holyoak; his sister Judy Holyoak; and his parents, Van Holyoak and Jean Holyoak. He is survived by: his children, Desiree Jade Holyoak Johnson, Daniel Quinn Holyoak, Amanda Dawn, Holyoak Johnson and David Neil Holyoak; and his siblings, Marcia Eisley, Ada Fowler and Joe Holyoak.
