It is with great sadness that we announce the death of Mathilda Ann Puchalski of Tempe, Arizona, who passed away on November 15, 2023, at the age of 91, leaving to mourn family and friends. She was predeceased by: her parents, Ladislaw Chonko and Margaret Chonko. She is survived by: her sister Helen Chonko; her children, Felicia (Eric Anderson), Theodore Puchalski Jr. and Gary (Heather Puchalski); and her grandchildren, Logan Anderson, Ethan Anderson, Zinnia Puchalski, London Puchalski and Declan Puchalski.
