He was predeceased by: his parents, Jason W. Hayward, Sr. and Lulu Bartlett Hayward; and his daughter Diana Hayward. He is survived by: his wife Wyn; his daughters, Debbie Hayward (John) and Julie Perry (John); his granddaughters, Jacqueline Perry, Jessica Perry and Jillian Perry; his nephew John Heidel, Jr. (Karen); and his great-nieces, Jennifer Heidel and Andrea Just. He is also survived by numerous nephews and nieces.
