He is survived by: his wife Diane; his children, Brandon Cook (Nastasia), Staci Arbogast (Ryan), Ryan Cook and Logan Cook; his grandchildren, Gwen Arbogast, Quinn Cook, Leona Cook and Norah Cook; his daughters, Tina Cook and Victoria Bernstein; and his siblings, Wendell Cook and Kristina Palmero. He is also survived by his ex-wives and mothers.
In lieu of flowers, donations may be made in his memory to the USC Norris Cancer Hospital Fund.
