He was predeceased by: his parents, Edmond John and Ethel Mae Forth (Nelson); his wife Dorothy Patricia Hogue of Marseilles; his son Lee Allen Forth; and his brothers, Jack and Roger Forth.
He is survived by: his children, Curtis Forth (Laurie) of Marseilles and Denise Meile (John Craig) of Lakeside, CA; his daughter-in-law Wendy Forth of Peoria, AZ; his grandchildren, Jackie Tomassi (Pat), Alex Forth (Torri) of Winter Garden, FL., Jessica Smith, Ryne Forth (Kelsey) of Marseilles, Caitlin McAllister (TJ) of Crest, CA and Micaela Meile (Steven Lounsbury) of Lakeside, CA; his brother Jerry Forth (Juanita) of Serena; and his close friend Barbara Clayton of El Cajon, CA. He is also survived by twelve great-grandchildren.
