He was loved and cherished by many people including: his parents, Edward John Hlavac and Lawanda Novalene Hlavac; his wife MaryLouise Dykzeul; his brothers, Michael and Gary; his sisters-in-law, LJ Dykzeul and Edie Hlavac; his nephew David Hlavac; his nieces, Jamie Schmaing, Michelle Hlavac and Elizabeth Davis; his aunt Thelda Lowery; his goddaughter Leslie Corcoran; and his children, Hans Erkelens (Netherlands), Kaja Dehli (Norway) and Hiroshi Inaba (Japan). He was also cherished by several cousins and grand nephews and nieces.

In lieu of flowers, donations may be made to Cal State Fullerton Philanthropic Foundation with designations to OLLI or the Theodore M. Dykzeul Scholarship Endowment for Jazz at the College of the Arts.
