She was predeceased by: her mother Beatrice Mathias Cunningham; her husband Charles "Chuck" W. Middlebrook; her daughter Melody Ann Middlebrook Hawksworth; her father Vance Cunningham; and her siblings, Minor Vance Cunningham, Sherman Cunningham and Joan Sanford.
She is survived by: her sons, Gregory L. Middlebrook (Donna) and Jeffrey W. Middlebrook (Sandy); her son-in-law Mark Hawksworth; her grandchildren, Seth Middlebrook (Sarah), Stephanie Middlebrook, Andrew Hawksworth (Carissa) and Matthew Hawksworth (Sue Zen Chew); her great grandchildren, Chloe, Tess Middlebrook, Luke and Caden Hawksworth; and her brother-in-law Bill Middlebrook (Juanita).
A funeral service will be held on Monday, June 9th 2025 from 11:00 AM to 12:00 PM at the Faith Bible Chapel (410 Fulton St, Salmon, ID 83467).
