He was loved and cherished by many people including: his parents, Jean Francis Cook and Aulaff Uzzia Cook; his wife Vicki Christina Cook; his children, Beverly Jean Cook (Lawrence Proctor) and Warren Lee Cook (Mary Jane Cook); his grandchildren, Amber Baxter, Kristen Jean Scott, Michael Jon Branch, Wanda Joy Kalebaugh, Diane Elizabeth Swords, Kevin Proctor and Christiana Delong; his brother Jesse Aulaff Cook; and his daughter Dawn Marie Cook. He was also cherished by great-grandchildren, nieces, and nephews.

In lieu of flowers, the family kindly requests that donations be made in his memory to St. Jude Children's Research Hospital.
