He was predeceased by: his mother Sally. He is survived by: his wife Hazel Jackson; his father Howard Jackson; his daughter Melissa Jackson; his sister Laura Smith (William); and his stepchildren, Matthew, Paul and Krysta. He is also survived by his extended family and wonderful friends.
