He was predeceased by : his parents, Paul Kubichko and Mary Kubichko (Bielinda). He is survived by : his wife Karen; his son Skye (Sarah); his grandchildren, Sophia and Samuel "Sammy"; his sister Carol Conely; and his nephew Ryan Renebome (Jennifer).

In lieu of flowers the family is asking for donations to be sent to: Parkinson's Foundation, American Cancer Society, or your favorite charity in remembrance.
