He was predeceased by: his parents, Maurice Clark and LaVonne Clark. He is survived by: his wife Annette Clark; his daughters, Emma and Sofia Clark; his brother Gary Clark (Diane Clark); his siblings' children, Melissa and Aaron; his sister Ann Jensen (Greg Jensen); his nieces, Sarah and Amy; his sister Becky Williams; and his siblings' children, Ethan and Elise. He is also survived by his family and friends.

The family respectfully requests that, in lieu of flowers, a tax-deductible tribute donation be made to either the American Heart Association or Venture Christian Church Outreach in David's name.
