He is survived by: his wife Tracy Jean Sly; his children, Alissa Danielle Sly Zamacona and Sean Austin Sly; his grandchildren, Tristan, Dante', Kenny, Priscilla and Colson; his son-in-law Andrew Zamacona; his granddaughter-in-law Abi; and his brother Mickey Sly. He is also survived by nieces and nephews.

In honor of his life, his family is asking those who wish to contribute to consider donating toward the costs associated with his passing. Any funds remaining after those expenses will be donated to the Juvenile Diabetes Research Foundation (JDRF) in his memory.
