He was predeceased by: his grandparents, Mark Crane and Deborah Lapka. He is survived by: his parents, Todd Kousky and Jennifer Kousky; his siblings, Paul Kousky (Jessica), Eric Kousky, Anna Kousky, James Kousky, Christopher Kousky and Eva Kousky; his godson Luke Kousky; and his grandparents, Patricia Crane and David Kousky (Mildred). He is also survived by many aunts, uncles, and cousins.
In lieu of flowers, donations may be made to the Archdiocese for the Military Services, USA, honoring Chad's life of faith, service and devotion.
