He was predeceased by: his parents, James Collins and Katherine Collins; and his significant other Martin Cogan, MD. He is survived by: his siblings, Eddie (Tracy), Mike (Michele), Joanne (Ed), Jimmy (Mindy), Kevin (Andi), Patti, Peggy and Fran; Billy. He is also survived by many nieces, nephews, cousins, and cherished lifelong friends.

Jack's adventurous life and impactful contributions to academia and the LGBTQ+ community left a lasting legacy.
